Heart failure monitoring
Weight monitoring is a crucial part of the heart failure care process.
Sudden weight gain shows that heart failure symptoms are
aggravating, and immediate action is required. Continuous weight
monitoring ensures that the symptoms are managed promptly pre-
empting the need for emergency hospitalisation.
How it works
Individual weight is monitored daily with minimum actions required
from the patient. Each patient is assigned weight thresholds by their
treating physician. All members of the treatment team have an
overview of the patient’s weight changes. They receive alerts if the
patient has not weighed herself or if the weight has changed and the
medication dose should be adjusted. In cases when acute help is
required, the team has all information to react immediately.
